D&D 5E What would a 'real' D&D society look like?

You're forgetting the possibility of mind controlling the populous - or at least key individuals - when done. A small group with Geas or Mass Suggestion can lock down a country just as easily as a large army.
Color me skeptical. The word "quisling" comes to mind. The strategy is more likely to throw a country into civil war than lock it down. (Admittedly, in certain circumstances, that might be all you need.)

You raise a very important question of level awareness. I think it might be worthy of a separate thread.

My current world has a high enough level awareness and despite it being a bit unsettling for the players at first it seems to work out. Levels = power, and power makes itself known unless special steps are taken to hide it.

I'd agree that level awareness would certainly be part of the issue. I'd also expect a sort of 'rumspringa' as a rite of passage for classed NPCs to acquire XP they need, and a professional career punctuated by sabbaticals for similar purposes. Assuming all party members get equal XP shares, this would create a whole industry of guides/bodyguards to escort people on levelling expeditions.

Well, even if the thread is necroed, here is a thought:

As powerful as a PC is, there is even easier power in being an NPC. They get all the powers that monsters have access too. In fact, it is ridiculously easy to become a monster in this edition. Consider the Bodak, all you really have to do is pledge your soul to a demon and kill yourself, and suddenly you can decimate entire towns just by looking at them.
