That's around. There's a monster in Spelljammer that has a brain extraction attack, if it takes a PC to 0 hp with it, that PC dies and their brain gets extracted. No save. Party got unlucky and I landed it on the Rogue. While running Light Of Xaryxis.
There others to IIRC, but it is rare. Probably best for most people/groups to play with it being rare. Of course we play death at 0 period, so no big deal for us!

It's interesting comparing this to the new statblocks that were previewed. I do love simple level based rules and I like simple ways to give monsters a range of levels, in principle I'm in favor of this approach. This is something I'd like more of so I can run it and do a comparison.

Definitely a cool post with some cool ideas. Love the idea about the fire elemental's aura gaining and losing strength as it takes cold or fire damage. That troll would be a great fight and I do like that there is a level range. The idea of seeing my players faces as they score a crit, only to see some troll bits animate and attack them would be priceless.

I'm totally describing that as "You hew his arm. Immediately he starts to grow a little stumpy arm - but wait! The fallen arm wiggles until it's up on the hand, and then it comes charging toward you, using its fingers for little legs and it's stump as a bludgeon!"
