I'm not sure that list exists, at least not 'officially'. TSR waa big on alphabetical lists. So much so that the follower tables roll numbers were altered to keep the lists alphabetical but to try and increase certain occurrences. It's why so many old bulletin boards had alternate tables as some of the first player made accessories (right after dice rolling programs).

Anybody's copy of the Monstrous Compendium, once they picked up a half dozen appendicies and tried to integrate them into a single binder.

That was one concept that probably needed a little more thought before they sent it to the printer
