Two headed creatures in Myth, Legend, and D&D

So I'm looking for names of creatures with two heads (the exact number of the counting, not more nor less) in myth, legend, and D&D. I've got only the short list below so far ... what am I missing?

Amphisbaena - two headed snake
Demogorgon - two headed demon
Ettin - two-headed giant
Janus - two-headed god

There's got to be a ton more. Isn't there a named two-headed dragon?

Wasn't Janus more a two-faced god than two-headed? As in, he had one face on the front of his head (to look forward to the future) and one face on the back (to look back into the past).
 


In the 3e Fiend Folio the two-headed dog is called a death dog.

Klurchir demons from there have a giant second mouth in their abdomen, so almost a second head.

Ghostly Visage is an undead parasite that can manifest as a transparent face over its host's.
 



By D&D do you mean just TSR/WotC or do you include things like Castle and Crusades' Goblin Hound Amung or the two-headed ogre mages which I believe are in the d20 Manual of Monsters for Warcraft?
 

Hydras, even the Learnean ones, could have as few as 2 heads.

The 3Ed Savage Species multi headed template starts with 2.
 

In some accounts, Cerberus, the hound of Hades, has only two heads. (He usually has three.) Also, his brother, Orthrus is a monstrous dog with two heads who was slain by Heracles.
