D&D 5E Stat-Block-style Character Sheet

Hi all! Here is my new character sheet in the "stat-block" style. All the really important info is on the first page, the second is blank for any extra info you want to record. Comments are welcome!

Some notes:
Under skills, the checkbox squares are used for expertise, not proficiency. If you list a skill, tool, etc,. you have proficiency.
Most (not all) larger text boxes are scroll-able, so you can have more in a list than what is shown at the moment.

REVISION BELOW:

  • added a second line below Class & Level for either a subclass or second class
  • added small text box for individual weapon proficiency
  • made Hit Dice box larger for multiple types (via MC) or for tracking used HD as well as max
  • separated larger text boxes on page 2 into narrower columns (numbers are there to shot location)

Under skills, I assume the filled in squares mean proficiency? If so, shouldn't the tool and weapon proficiencies be filled in? Also, how do you denote expertise for thieves?

The hit dice is missing in the example and in the box for hp, I would align the # to the left or right of the box.

Finally, that's way too little space for equipment and treasure for an adventurers. ;)

I think it's for Expertise (it's a bard). It looks like you should only list those things you have proficience for and can mark special skills (like expertise).
 

Do you tell your players which style of character sheet to use?

When I DM I do. I just like for the players all to use the same sheet. I find one that I think has all the information we need as a group and its just easier if everyone is on the same page to locate information quickly.

Why?

Shouldn't each person use the sheet they personally find easiest? We have people using official sheets, some using apps, others using notepaper. Each does it the way it suits them.

Maybe the DM wants to have a copy of all the character sheets, for reasons that don't matter, and it's easier for him to do whatever he needs the character sheets for if they're all laid out the same. I'm playing in a game where the GM strongly requested that we all use a computerized character thing, and I loathe and detest it but I'm using it for that campaign (and that campaign only).
