There are a set of magic items that allow a reroll for a specific thing once/day. I want to say they are from Eberron, but I honestly don't remember. Like a belt that allows you to reroll an attack 1/day. They are all cheep and all require that you have worn the item for 24 hours before they work....
 


IIRC, a Wu-jen class feature allows you to reroll initiative once per day.

The lucky weapon enhancement allows you to reroll an attack once per day.

Blind-Fight: Reroll miss chance for concealment; PH 89
Deadly Precision: Reroll 1s on sneak attack dice; XPH 45
Guardian Spirit: Reroll initiative two times per day, reroll any save 1/day; CA 80

I should point out that guardian spirit requires the 1st-level Wu-jen ability (increasing it to 2/day and providing for the rerolling of one save, rather than simply giving a flat-out 2/day initiative and one save reroll.) It would be pretty broken if it did not have that requirement! :p
 

The binder allows rerolls if you bind Balam, which can be done at 10th level (8th with improved binding). You can reroll any one attack, saving throw or skill check once every five rounds.

You see the many alternative branches that reality could take in the next few seconds, and with this foreknowledge you gain the ability to reroll one attack roll, one saving throw, one ability check, or one skill check each round. You must take the result of the reroll, even if it’s worse than the original roll. You do not have to make another roll if satisfied with your original roll.

You can manifest this power instantly, quickly enough to gain its benefits in an emergency. Manifesting this power is an immediate action. If you use the power to reroll a saving throw, you can manifest this power even when it is not your turn.
