I know there are plenty of good character sheets out there. Does anyone know where I can find a blank form for building a monster?

Seriously though, the good ol' college-ruled never goes out of style. Sometimes I just type it up on the computer and print out a new one every level.

The initiative cards from The Game Mechanics kind of function as a mini character sheet.

There are character cards and monster cards. I have yet to use them for initiative but have many of my NPCs and nameless faceless thugs on them for ease of finding. There are both modern and fantasy versions.

There were a couple on RPGsheets.com. I searched for both NPC and Monster sheets and found some of each. Some of them hurt the eyes, so beware.

It's intended more to help editors than to help DMs, but it's been useful to me when I make encounters and NPCs for my game.
