Has this creature ever been updated? There seems to be something funky in its sizing. It's listed as a Medium creature, but its Space in the 3.5 update is 10 ft., the advancement line starts at Large, and all of the text describing its tactics seems to indicate a fairly big creature.

Also, should it have the extraplanar subtype?

Not according to the Wizards monster index, so the current version would be the one in the Update booklet.

That appears to be a mistake that goes back to the 3.0 version that had a space of 5 by 10. It should be 5 by 5 in both versions under normal circumstances.

As far as I recall, that isn't all that rare. Odd, but not unprecedented.

and all of the text describing its tactics seems to indicate a fairly big creature.

The rearing up is pretty bear-like (who also start at Medium), but the bull rushing seems to be a less optimal tactic for a creature with Strength of 11.

Also, should it have the extraplanar subtype?

Not according to the flavor text, which makes it an inhabitant of the Material Plane, much like a white dragon.
