In another thread, I mentioned parenthetically:

(As an amusing anecdote, I once ran a d20 Modern game set in Tolkien's Seventh Age -- a modern urban fantasy game set in Tolkien's mythos. It was fun.)

Forum member Achan hiArusa then responded:

Well, Achan, ask and ye shall receive. I had to go digging through tons of boxes in my mother's garage, but I found them, from nearly 8 years ago. I give you:

Legend of Modern-earth: A d20 Modern Urban Arcana Roleplaying Campaign

The Setup: The game took place in 2004 (both IC & OOC) in Tolkien's Seventh Age. The continents of the world had foundered and shifted under the scouring seas, and Middle-earth had long made way for Modern Earth. Melkor, Sauron and the forces of evil lay imprisoned in the Outer Void beyond the Circles of the World...but not for long; magic would soon return....

The Players and their Characters:
Matt was going to play Zeppel the Elf, but he bagged shortly before the campaign started, and I had banked on having an Elf so...
Scott, who was going to play Luke Hastram, from the Horse Lords motorcycle gang, agreed to play the Elf, and created Leonidas.
Steve played Gymbylwyk the Hobbit.
Mike played Jacob (the character was originally named Simon, so you may see that in some of the early notes), a former Army Ranger who finds out he is actually... [sblock=spoiler!...]...the second coming of Túrin Turambar![/sblock]

We used the Urban Arcana supplement for WotC's d20 Modern system. Unfortunately, we didn't get to finish the campaign, but we made it through several months of weekly sessions. You can download my game notes, complete with appendices, from my MediaFire account here.
