Hunger of Hadar question

I'm still trying to figure out how the Warlock power Hunger of Hadar works.

According to the power it does an automatic 2d10 damage to any creature that starts it's turn in the hunger cloud or enters the hungers cloud. Plus when the Warlock sustains it with a minor action she can do an attack against every creature in the cloud. It also blocks line of sight.

What I'm wondering is does it do anything when the Warlock first casts it? Lets say there are three enemies in the cloud when the Warlock casts it. Does she get to attack them or can she only do that when she sustains the power? If so then if I'm reading this right then any enemy that gets pushed out of the cloud before the start of their turn wouldn't take damage from the cloud at all. Is that right?

Also what does "block line of sight" actually mean? Does that mean it grants Total Concealment to the people inside the cloud? or does that only mean people on one side of the cloud can't target people on the other side or inside the cloud? I'm confused about this power.

The zone doesn't do anything when the warlock casts it. At the start of those 3 enemies turns, each one is going to take 2d10 damage (if they are still inside, so if they are pushed out they don't take damage). When it's the warlock's turn again and he sustains the zone, he can make the secondary attack.
Regarding "blocks line of sight" it means that you can't see what's inside or on the other side of the zone, so enemies have total concealment (i'm not sure if you have to guess in which square they are in order to attack them or not, because it's a rule that i don't use in my games so i don't remember)

This came up in our game in relation to stinking cloud.

Line of sight works by tracing a line from any corner of your square to any corner of the target square (PHB page 273 and 281). The zone is made up of multiple squares that you may have to look at separately for some purposes. If a square blocks line of sight, you can't get line of sight through or along the edge of the square, but you do have line of sight to the nearest corners of the square, so people in the first rows of squares at the edge of the cloud are seen and can see out normally.

If you don't have line of sight to a square, you can't see or target creatures in it (you can target the square) and they have total concealment to you.
