More issues (I'll edit this post as I find them):
Crawling Claw: We are told to change it to swarm, but little else. The distraction DC should be 10. Swarm damage is not given, but should probably be 1d6. The Str score should probably be reduced to 1. The organization entry is no longer correct.
Demon, Ghour: Should have added "A ghour's natural weapons, as well as any weapons it wields, are treated as chaotic-aligned and evil-aligned for the purpose of overcoming damage reduction." Change teleport without error to greater teleport. The save DC for the roar ability should be 21 (10 + 1/2 HD + Con modifier), not 13.
Demon, Yochlol: No damage reduction or overcoming damage reduction info. Change teleport without error to greater teleport. Does not mention to add to SQ line: resistance to acid 10, cold 10, and fire 10. (Although they had fire resistance 20, and didn't mention any changes to that.)
Helmed Horror: Should have +20 hp as Medium construct. Grapple bonus should be +12. Should not have feats or skills, as it is mindless.
Tomb Tapper: Should have +40 hp as Huge construct. Change "blindsight" to "blindsight 120 ft." on SQ line. Add detect magic, low-light vision, stone shape, and tremorsense 60 ft. to SQ line. Change the attack lines from "warhammer" to "maul". Change "spike stones effect like a 5th-level arcane spellcaster" to "spike stones (caster level 5th)", since spike stones is not an arcane spell.
Tressym: Incorrectly listed between "Unicorn, Black" and "Wemic" in Monsters of Faerun section. Should be located in Forgotten Realms Campaign Setting section.
