Starspawn is what they are calling at least some of the Great Old Ones now. They are also tied to Foulspawn somehow, but that was confusing. I'll rewatch it.

Okay rewatched, I'm still confused on how foul spawn, star spawn, and Elder Evils will actually work in the 5e version of Star Spawn. Are they going to be a type of Great Old Ones, the offspring of Great Old Ones, or are more like Foulspawn?

Okay, I literally can't watch the embedded video, but to the best of my knowledge, this is the entymology of the different terms:

Foulspawn: A humanoid creature transformed into a grotesque Aberration by exposure to the energies of the Far Realm. Originated in 3rd edition, to the best of my knowledge.

Elder Evil: A god-like Aberration or unique Outsider, possessing apocalyptic-tier powers and influences, specifically created to invoke the "feeling" of Mythosian-style deities in D&D. Originated in 3rd edition.

Starspawn: Sapient stars & planets with ambiguous ties to the Far Realm - being specific, Starspawn refers to the unique entities made by these sapient stars and unleashed on the mortal world, but it is also used for the stars themselves, such as Hadar and Allabar and Atropus. Originated in 4th edition.

Great Old One: Nebulous term that encompasses both Elder Evil, Starspawn, and actual migrants from the Cthulhu Mythos. Originated in 5th edition.

"Chris Perkins (@ChrisPerkinsDnD) and Matt Sernett (@Sernett) delve into the complex lore of the inscrutable Star Spawn. These abhorrent beings are heralds and foot soldiers from the realms of the fiendish terrors known only as the Elder Evils and will now be available for your campaigns with the soon to be released Mordenkainen’s Tome of Foes, available everywhere May 29th!"

This clarifies the relationship between Elder Evils and Star Spawn, Star Spawn are the heralds and soldiers of the Elder Evils."
