In Chains of Avernus the PCs (levels 11-20) are pursued by a pair of Devil brothers that are both CR 21 and you must kill both in the same round to finish them off. It can be a very dangerous conflict that could occur at any point in the adventure.

IDK, we play 5e pretty much the same way we played 1e 40 years ago. For me, as the DM, it is a lot easier to run a 5e game than a 1e game and the art is better. I also like design of monsters better in 5e (generally).

We only know that some people or person wanted it. We do know a major person who expressed those desires left the company to start his own early on during the design process (before the playtest IIRC). So we do don't know how much the overall design team wanted that or how strong they wanted it.

I am not sure I agree with your statement, but I am curious. What holes are you talking about? I ask primarily because whatever perceived holes 5e has I feel they have been filled by 3pp (or my own homebrew as no one does armor as DR correctly IMO).

Any DM can kill a PC or get a TPK with trivially little effort. The art of the DM is an making a challenging, interesting, exciting, and engaging encounter that doesn't lead to a TPK.
