I don't call for initiative until something happens to trigger it. If the party have snuck up and are watching the baddies unobserved, they get an action to trigger the combat and then we roll for init.

2nd edition had my favourite psionic system but even that had balance issues. Dark Sun handled it well but I think that's simply because it was built into the setting. I'd like psionics to use a power point system and have different "spells" to the standard classes where possible.
